This June, the Government of Ontario decided that all players (parents if under 18), team officials, and on-ice officials are required to review concussion awareness resources and sign an acknowledgement form.
The Resource Package and Acknowledgement Form are available below.
Novice Coaches will have printed copies of the Acknowledgement Forms for Parents at the 2 remaining pre-skates. All Novice players will be required to have this form signed to participate in hockey this year.
